The amulet is in the shape of a stylized leaf, with two hooks on its edge for attachment to a chain, engraved entirely with Kabbalistic letter combinations, including the verse: ”Ben Perath Yosef Ben Perath Ali Ein Banut Ceda Ali Soor”.
