The recorder is constructed with a brushed aluminum faceplate set within a wooden veneer enclosure. It features a three-head configuration for real-time monitoring and includes dual analog VU meters alongside independent mic and line level controls for both left and right audio channels. The front panel bears a black nameplate reading AKAI 4000DS Mk-II and is marked MADE IN JAPAN near the left reel hub.
