Printed bilingual form in Polish and German recording a birth from 1913, bearing manuscript entries, official signatures, registry markings and an original Austro Hungarian revenue stamp of 1 Krone depicting Emperor Franz Joseph. The document represents the civil and communal registration system of the Jewish population of Galicia during the final years of the Austro Hungarian Empire.
