The partially printed document, completed in typescript, was issued by the American Federation of Musicians and is a contract between Lewis and his five band members and Ronald Boleyn, in which the musician and his band agree to perform two shows at the Club Angel, Fort Wayne, Indiana, on 15th August 1966 at 9pm and 2am for a fee of $750, the employer also agreeing to provide a piano and public address system in good working condition.
