Zichron Yosef by Rabbi Chanoch Henich Pack, one of the Dayanim of Kashoy - Bardejov, 1929.  Zichron Yosef on Shulchan Aruch Orach Chaim and at its end a completion Kuntres written by the author after the book was published.
