פני המדליה:
הסמל הרשמי של חגיגות שנת העשרים וחמש – מנורה, וסרט היוצר את הספרה 25 ועליו כתובת "ישראל". על השפה, למטה, כתובת באנגלית "שנת עשרים וחמש לישראל".
גב המדליה:
שנים עשר המזלות, מתוך הפסיפס שנתגלה ברצפת בית-הכנסת מהמאה השישית בחפירות בית-אלפא.
היקף: סמל המדינה, והמילים "מדינת ישראל" בעברית ובאנגלית. "כסף 935" ובאנגלית "סילבר".
