[4], צד, [4] דף. 30 ס"מ. דפים בודדים כהים במיוחד. מצב בינוני. כתמים, בהם כתמי רטיבות. בלאי וקמטים. סימני עש (במספר מקומות סימני עש קשים), עם פגיעות בטקסט. חותמות ורישומים בכתב-יד. כריכה ישנה ופגומה (צדה הקדמי שבור), עם סימני עש.
