Printed by Rabbi Moshe Av Beit Din of Slavita.
Due to blemishes to the title page, the year of printing is unknown precisely. The approbations were given in 1819; thus, presumably the Chumash was printed in the year.
The approbations are by the Gaon Rabbi Mordechai Margaliyot Av Beit Din of Satanov and by the Dayanim of Ostroh.
