Rav Naftoli Carlebach, Av Beis Din of Baden (5649-5728) son of Rav Shlomo Carlebach Av Beis Din of Lubeck, from 5690 Av Beis Din of Baden, Austria. After Austria's annexation to the Nazi empire he escaped with his family to Lithuania and from there to the United States. The dedication is written on Sefer Toras Chaim by Rav Chaim of Kosov.
