132, 65 p. Pages 1-4, 40-45, 83 - missing and completed with photocopy of pages in an ancient and original color.
Good condition-. Tiny tears on some pages with no text missing. Wear at the margins of some pages. Slight lack of text on last page and other individual pages. Slight moisture stains. New cover.

בנוסף נדפסו עמו תפלה לחולה, תפלה ליולדת המקשה לילד, יהי רצון לאחר כל ספר שבתהלים לליל הושענא רבא, תפלת פדיון נפש כפי הרמב"ן, תפלת אבות על בנים, תפלה בבית האבל על הנפטר.
קלב, ס"ה דף. דפים א'-ד', מ'-מ"ה, פ"ג - חסרים והושלמו בצילום בדפים בגוון עתיק ומקורי.
מצב טוב-. קרעים זעירים בחלק מהדפים ללא חסרון טקסט. בלאי בשולי חלק מהדפים. חוסר קל בטקסט בדף אחרון ובדפים בודדים נוספים. מעט כתמי רטיבות. כריכה חדשה נאה.
